Hospital-to-Home Care in San Diego

Non-medical support for the first days and weeks at home

Help with personal care, meals, reminders, and daily routines when a loved one comes home from the hospital or a rehab stay—provided by caregivers Better Choice Care LLC carefully screens and employs.

What It Is

The First Days at Home Matter Most

Going home is a relief, but it can also be hard. New medicines, less strength, and a long list of instructions.

Non-Medical Help After Discharge

Many families are working and can’t be there all day. A caregiver can help your loved one settle in and follow the plan their care team gave them.

Support follows the written service plan and is adjusted as everyday needs change.

How It Works

Our Hospital-to-Home Plan

Care is built around your loved one’s written service plan, schedule, and caregiver availability.

Before Discharge

  • Free consultation with the family
  • Review the schedule and daily needs
  • Talk about home setup, like walker space or a shower chair
  • Plan the first caregiver visits

The First 72 Hours

  • A caregiver can be there when they arrive home
  • Help with bathing, dressing, and moving safely
  • Medication reminders (not giving medicine)
  • Simple meals and help staying hydrated
  • Share any concerns we notice with the family

The Weeks After

  • Reminders for exercises the therapist gave
  • Help getting to follow-up appointments, when part of the service plan
  • Light housekeeping, laundry, and errands
  • Companionship so they don’t feel alone

Hospital-to-home care is non-medical. Caregivers do not provide nursing, medical treatment, wound care, therapy, or medication administration. Transportation assistance may be available only as part of a scheduled care visit and written service plan. Better Choice Care does not provide standalone, medical, or wheelchair transportation.

FAQ

Hospital-to-Home Questions

Is hospital-to-home care the same as home health care?

No. Home health is medical care from nurses and therapists, usually ordered by a doctor. Better Choice Care provides non-medical help with daily life, like bathing, meals, reminders, and companionship. Many families use both at the same time.

Can care start the day my parent comes home?

Start timing depends on the consultation, the schedule you need, and caregiver availability. Call us as early as you can, even before discharge, so we can plan together.

How long does hospital-to-home care last?

It’s up to your family. Some people need help for a week or two, others longer. We do not require a long-term commitment. Service terms and any minimums are explained before care begins.

Do your caregivers give medicine?

No. Our caregivers provide medication reminders only. They do not give medicine or provide medical care.
